Private MBBS India vs Abroad: The Real Cost in 2026

Official fee orders and university fee schedules, side by side, so a family can see what a private Indian seat and a European medical degree really cost across the whole course.

Private MBBS India vs abroad is a cost question with one variable: the quota. A general quota seat at a private medical college in India costs roughly Rs. 36 lakh to Rs. 42 lakh in tuition over four and a half years. Six years abroad runs about Rs. 86 lakh to Rs. 1.60 crore.
On an NRI or management seat, the India figure overtakes Europe. That surprises most families, so the rest of this page shows the working.

What does a private medical college in India actually charge?
India does not have one private fee. It has three, and which one a student pays depends entirely on the quota they enter through.
The clearest official picture comes from Kerala, where the Commissioner for Entrance Examinations publishes the approved fee for every self-financing medical college. For the 2026 admission cycle the 2025-26 figures are being charged provisionally while the Fee Regulatory Committee finalises the new year.

Deemed universities sit in a band of their own because they set fees nationally rather than under a state committee. SRM Institute of Science and Technology publishes an NRI fee of USD 50,000 a year, a one-time registration charge of USD 300, and a stated total of approximately USD 275,300 for the course and internship together. At the European Central Bank reference rate of 18 September 2026, that is close to Rs. 2.64 crore.
So the range within India, before anyone has left the country, runs from about Rs. 36 lakh to about Rs. 2.64 crore.
The four and a half year rule that changes the India number
Most fee comparisons multiply an Indian annual fee by five or six. That is wrong, and it inflates the India side by several lakh.
A Kerala government order issued on 17 July 2026 directs that MBBS fees be charged only for the prescribed academic duration of four and a half years, not for the compulsory rotating internship year. The academic course is 4.5 years. The internship is the twelve months after it. Under that order, the internship year carries no tuition.
Now look at the SRM figure again. USD 50,000 a year multiplied by 4.5 years plus the USD 300 registration comes to USD 225,300. The published total is USD 275,300, which is exactly 5.5 years of the annual fee. The internship year is billed.
That single difference is worth about Rs. 48 lakh on an NRI seat. Before a family signs anything, the question worth asking the admissions office is plain: how many years does this fee cover, and is the internship year included?

What do European medical universities charge?
European fees are published openly on university websites, in euros, for the full six-year programme. There is no quota system and no separate NRI rate, so the number a family sees is the number it pays.

Two of these universities, UPJŠ Košice and Rīga Stradiņš University, charge less across six full years than a Kerala NRI seat charges across four and a half.
Private MBBS India vs abroad: the full six-year cost
Tuition is only part of the answer. A student in India on a general quota seat usually lives at home or in a college hostel and earns a stipend during the internship year. A student in Europe pays rent and food for six years.
Living costs are genuinely country-specific, so the honest way to do this is one worked example rather than one blended average. Latvia publishes a clear official figure: the State Education Development Agency puts student living costs at EUR 700 to EUR 800 a month including accommodation.

Hungary comes out cheaper on the living side than most families expect. Study in Hungary, run by the Hungarian government's Tempus Public Foundation, prices a Budapest university dormitory at 36,000 forint a month, close to EUR 99, with Szeged at 13,000 forint.
The verdict on tuition alone is straightforward. A general quota private seat in India is the cheapest route on this page. An NRI or management seat is not.
What each side costs that nobody puts in the table
These are the lines that move a budget after the decision is made.
On the India side
Hostel and mess are charged on top of the approved tuition and are not covered by the fee order, so they have to be asked for college by college.
The internship year carries no tuition in Kerala, and interns are paid a stipend, which pulls the effective cost down rather than up.
Fees stay provisional until the Fee Regulatory Committee signs off, so the figure quoted at admission can be revised within the same year.
A student who misses the general quota by a few marks moves to a quota that costs two to three times more, and that jump happens inside the same college.
On the Europe side
Deposits and one-time charges arrive before tuition does. Semmelweis University alone asks for a USD 400 admission procedure fee, a USD 2,000 deposit and a USD 230 enrolment fee.
Health insurance, residence permit renewal and apostille of documents recur every year rather than once.
Flights home, usually once or twice a year for six years.
The rupee to euro rate is itself a cost. A five per cent move against the rupee on a EUR 81,000 course is about Rs. 4.45 lakh.

How to compare the two numbers properly
Most families compare an Indian annual fee against a European annual fee and stop there. Four checks turn that into a real comparison.
Ask the college how many years the fee covers, and whether the internship year is billed. Kerala's order sets four and a half years, while SRM Institute of Science and Technology bills five and a half.
Add hostel and mess to the India figure, and rent and food to the Europe figure. Neither sits inside tuition, and both run for the length of the course.
Convert the European figure at the rate on the day you decide, then add a margin for currency movement across six years.
Price the quota you actually hold, not the one you hope for. General quota and NRI quota sit in the same college and differ by two to three times.
Who should choose which
If a student secures a general quota seat in a state private college, that is the cheapest path to an MBBS on this page and the arithmetic is not close.
If the only Indian seat available is an NRI or management seat, the comparison flips completely. Rs. 97 lakh to Rs. 2.64 crore in Indian tuition buys a full six years in Slovakia or Latvia, living costs included, with money left over.
The scale of the squeeze is worth stating plainly. The National Testing Agency declared on 16 July 2026 that around 20 lakh candidates appeared for NEET UG 2026 and 11.21 lakh qualified. The health ministry placed on record on 13 February 2026 that India added 48,563 MBBS seats between 2020-21 and 2025-26. Both things are true at once: there are far more seats than there were, and far more qualified students than seats.
Two points sit outside the money. A degree from a European university means returning to India through NExT or FMGE before registration, and that is a real step, not a formality. Recognition is the other. The World Federation for Medical Education does not accredit individual medical schools, it recognises the agencies that accredit them, and it publishes which agencies hold that status.
