Senior residency is the post you take after MD or MS, and it is capped at three years. At a central institution it pays Level 11 of the pay matrix, a minimum of Rs. 67,700 a month. One year of it is what makes you eligible to become an Assistant Professor.
Senior residency after MD or MS: key takeaways
Senior residency is a tenure post, not a faculty post. The National Medical Commission says so in plain words, and the distinction decides your pay, your title and your promotion path.
Three years is the hard ceiling. The cap counts every senior residency you have done anywhere in that speciality, so a second post does not reset the clock.
One year is the number that matters. Assistant Professor in a broad speciality requires one year as a senior resident after the MD or MS, which is why most people do at least that.
Central institutions pay Level 11, a minimum of Rs. 67,700. Non-Practising Allowance is added on top, and the same level appears across central and state college adverts.
Age is the quiet cut-off. Adverts set 45 years at the time of appointment, with the usual category relaxations and nothing for time lost.
In some states senior residency is the bond. Maharashtra allots senior residency posts to postgraduate degree holders through bond service counselling rather than by open advert.
What is senior residency after MD or MS?
A senior resident is a doctor doing residency in a department after finishing a postgraduate degree. The Teachers Eligibility Qualifications in Medical Institutions Regulations, 2022 define it in exactly those terms.
The same regulations add two lines that most pages skip. Senior residency is a tenure position not exceeding three years. And senior residency posts are not faculty positions.
That second line is the one to hold on to. A senior resident works in a teaching department, teaches junior residents and runs a unit's daily patient care. None of that makes them faculty. Faculty means Assistant Professor and above.
If you are still choosing a postgraduate subject, our NEET hub carries the counselling and seat material for that stage. Our guide to how to become a doctor in India sets out the route up to it.
Senior resident or junior resident: which are you?
The two words are used loosely in hospitals and precisely in the regulations.
The Post-Graduate Medical Education Regulations, 2023 settle it. A senior resident holds a postgraduate degree in the broad speciality. Students doing a broad speciality course are junior residents. Students doing a super speciality course are also called senior residents.
So the title covers two different people. One has finished an MD or MS and taken a paid post. The other is a DM or MCh student still in training. Both are called senior residents, and only the first is applying for a job.

Who is eligible for senior residency?
Condition | What the adverts ask for |
|---|---|
Degree | MD, MS or DNB in the department you are applying to |
Registration | Registration with a State Medical Council |
Internship | Compulsory rotating internship completed |
Age | 45 years at the time of appointment |
Prior tenure | Three years of senior residency already done makes you ineligible |
Current employer | No Objection Certificate, or resignation accepted before joining |
The age relaxations follow the usual pattern. The AIIMS Rewari senior resident advertisement of September 2026 gives three years for Other Backward Classes candidates and five for Scheduled Caste and Scheduled Tribe candidates. Candidates with benchmark disabilities get ten years, and ex-servicemen get five more.
Senior residency tenure and the three year cap
Three years is the limit, and it is cumulative.
The AIIMS Rewari advertisement states that total senior residency, including any earlier service in the same speciality, cannot exceed three years. The Government Medical College and Hospital, Chandigarh senior resident application goes further and rules out anyone who has already completed three years.
Two practical consequences follow. Moving between institutions does not buy you extra time. And once the three years are used, that door is shut for good, so the posts you take early decide what is left later.
Tenure is not always granted in one block. At AIIMS Rewari a review committee assesses performance after the first year, and an extension depends on what it recommends.
How senior residency posts are advertised
There is no single national cycle, which is why people miss them.
Central institutions increasingly use a rolling advertisement. The NEIGRIHMS rolling advertisement for senior resident posts works on repeating cut-off dates rather than a single closing date. Its first cut-off falls 21 days after publication in Employment News, and later cut-offs are announced only on the institute website.
Read that condition again, because it is the trap. Changes to vacancies, eligibility and terms are posted on the website and not in any newspaper. If you are waiting for an advert to appear in print, you will be late.
Rolling adverts. One notice, several cut-off dates, with fresh vacancy lists published after each cycle closes.
Walk-in notices. A date, a room number and a deadline on the morning itself, common at state and union territory colleges.
Bond allotment rounds. In some states the posts are filled through counselling rather than advertised individually.
Departmental notices. Single-department vacancies posted on the institute site between larger cycles.

How the senior residency selection process works
For most posts there is no written examination at all.
The AIIMS Rewari advertisement says selection is purely on an interview of 100 marks. Where two candidates tie, the older candidate is placed higher, which is an unusual rule worth knowing. NEIGRIHMS screens applications first and calls only eligible candidates for a walk-in interview.
The Chandigarh route is faster and harsher. The walk-in is held on a stated date. The form and the original certificates have to be handed in by a fixed hour that morning, and the fee is paid on the spot.
Step | What happens |
|---|---|
Application | Form with certificates, by post, online or in person on the day |
Fee | Rs. 1,500 general and Other Backward Classes, Rs. 1,000 Scheduled Caste, Scheduled Tribe and Economically Weaker Sections at AIIMS Rewari |
Screening | Eligibility checked against the cut-off date for that cycle |
Interview | Usually the only assessment, carrying the full marks |
Joining | Within 15 days at AIIMS Rewari, or the selection lapses |
How to apply for senior residency, step by step
The order matters, because two of these steps take weeks and the rest take a morning.
Find the cut-off date for the current cycle. On a rolling advertisement this is published on the institute website, not in a newspaper.
Check yourself against the two hard limits. Senior residency already served in that speciality, and your age at appointment.
Get your No Objection Certificate early. If you are already working, this is the step most likely to delay you.
Fill the prescribed form and pay the fee. Some institutes take a demand draft rather than an online payment.
Carry every original to the interview. Degree, registration, internship completion, category and identity documents.
Join inside the stated window. AIIMS Rewari allows 15 days from the selection letter, after which the selection lapses.
Senior residency stipend and pay
This is a salary, not a stipend, and it is published.
NEIGRIHMS sets senior resident pay at Level 11 of the pay matrix with an entry pay of Rs. 67,700 a month, plus Non-Practising Allowance and other allowances admissible under central government rules. It names the arrangement as the Government of India Residency Scheme. AIIMS Rewari gives the same Level 11 figure for medical candidates.
Chandigarh matches it. Senior residents there are paid the minimum of Level 11, Rs. 67,700, plus 20 per cent Non-Practising Allowance.
Compare that with what the same doctor earned a year earlier. During the MD or MS the pay is a stipend, and our post on PG resident stipends in India and the stipend web story set out what colleges actually pay. Senior residency is the first time the figure is a pay matrix level rather than a college decision.
Level 11 also sits above Level 10, which is where a doctor with no postgraduate degree enters central government service. The gap is roughly 21 per cent at the minimum, before allowances.

What a senior resident actually does
The job description is written down, and almost nobody reads it.
The Directorate General of Health Services hospital manual sets out the duties in a central government hospital. Senior residents of each speciality must be physically present in the emergency at all times. They examine every patient and start treatment.
Medication reconciliation. Recording what the patient already takes, so that doses are not duplicated and drugs do not interact.
Escalation. Consulting a specialist for every seriously ill patient, and arranging transfer to the ward, theatre or intensive care.
Records. Ensuring the history, examination, investigations, provisional diagnosis and care plan are written on the case sheet.
Medico-legal work. Helping the casualty medical officer complete medico-legal records.
Supervision. Junior residents carry out treatment as advised by the senior resident, and do minor procedures and dressings under that supervision.
Read as a list it looks administrative. In practice it means the senior resident is the most senior doctor physically in the building for much of the night. Our page on the 2023 resident entitlements covers what the regulations guarantee in return.
Central institution or state college: the real difference
Feature | Central institution | State or union territory college |
|---|---|---|
Pay | Level 11, Rs. 67,700, plus Non-Practising Allowance | Level 11, Rs. 67,700, plus 20 per cent Non-Practising Allowance at Chandigarh |
How posts open | Rolling advertisement with repeated cut-off dates | Walk-in interview notices, often at short notice |
Term offered | Up to three years, reviewed after the first | Six months at Chandigarh, extendable until a regular incumbent joins |
Selection | Screening, then interview | Walk-in interview on the stated day |
Termination | On the terms in the advertisement | Services can end if work or conduct is unsatisfactory |
The money is not the difference. The security is. A central post offers a term you can plan around, while a six month extendable post ends whenever a permanent appointee arrives.
The senior residency bond, and where the post is the bond
Central senior residency advertisements carry no service bond. Neither the AIIMS Rewari notice nor the NEIGRIHMS notice mentions one.
Some states work the other way round. In Maharashtra, postgraduate degree holders who signed a bond serve it as senior residents, and the posts are allotted through counselling. The Directorate of Medical Education and Research publishes the rounds on its bond service notice page, including vacancy lists titled as senior resident vacancies for bond allotment.
The state is strict about it. Its government resolutions on bond service include one of January 2017 treating failure to perform bond service as misconduct under the Maharashtra Medical Council Act. Another, of February 2017, says service done outside Maharashtra does not count.
So the answer to "is there a bond" depends entirely on which door you came through. Applied to a central institution, no. Graduated from a state college that bonded you, the senior residency may be the bond itself.

Why one year of senior residency matters most
If you remember one thing from this page, make it this.
The 2022 regulations set one experience condition for Assistant Professor in a broad speciality. It is one year as a senior resident in that subject, after acquiring the MD or MS, in a recognised or permitted medical college.
That single line explains the whole queue for these posts. Without the year, an academic career does not start. With it, you are eligible for the first faculty rung, and the three year cap exists precisely because the post was never meant to be permanent.
It also explains a common mistake. A senior residency in a hospital that is not a recognised or permitted medical college pays the same and teaches you plenty, and it does not meet the condition.
Senior residency, super speciality or practice: how to choose
Route | What it costs | What it opens |
|---|---|---|
Senior residency | One to three years on Level 11 pay | Eligibility for Assistant Professor, and a stronger super speciality application |
Super speciality | Three years of DM or MCh, on a stipend again | A narrower speciality and the faculty ladder inside it |
Consultant or practice | Nothing in years | Earning sooner, with the academic route effectively closed |
Government medical officer | A recruitment cycle | A permanent post rather than a tenure one |
Senior residency and the super speciality are not rivals. Many candidates do one or two years of senior residency while preparing for the super speciality entrance, because the post pays and the preparation needs time. Our comparison of INI CET against NEET PG and our guide to which course is best after MBBS weigh the next stage.
Who cannot apply for senior residency
Anyone who has already done three years. The cap is cumulative across institutions in the same speciality.
Anyone over the age limit. Forty-five years at appointment, with category relaxations and nothing else.
Anyone not yet registered. A State Medical Council registration is required, not merely the degree.
Anyone who cannot be released. A serving doctor needs a No Objection Certificate, or an accepted resignation before joining.
Anyone whose result is late. Qualifications must be complete by the cut-off date stated for that cycle.

What if you do not get a senior residency post
The posts are not evenly spread, and popular departments at popular institutes fill quickly.
Apply to the next cut-off. A rolling advertisement runs repeatedly, so a missed cycle is weeks rather than a year.
Widen the geography. Smaller and newer institutes advertise more often and attract fewer applications.
Take a state post for the year. A six month extendable post still counts towards the one year that faculty eligibility needs, provided the college is recognised or permitted.
Sit the super speciality entrance instead. A DM or MCh seat makes the senior residency question irrelevant for three years.
Look at government service. Our page on government jobs after MBBS covers the permanent posts, and NEET PG counselling covers the seat route.
Consider licensing exams abroad. The United States route runs through the USMLE, and the United Kingdom through the General Medical Council's PLAB test, each with its own registration system.
Senior residency after MD or MS: the bottom line
Senior residency is a short, well paid, deliberately temporary post, and it is the only bridge to a faculty career.
Treat the three years as a budget rather than a plan. Spend one year to unlock Assistant Professor eligibility, spend a second if you are preparing for a super speciality entrance, and keep the third in reserve. The cap does not refill.
