Army Doctor Salary 2026: Pay by Rank, MSP and Allowances

The army doctor salary in India is the pay of a medical officer in the Armed Forces Medical Services (AFMS), and it is built from three parts: basic pay from the 7th Pay Commission's defence pay matrix, a Military Service Pay of ₹15,500 a month, and a Non-Practising Allowance of 20% of basic pay, with Dearness Allowance added on top.

In plain numbers, a Captain starts at a basic pay of ₹61,300. Add Military Service Pay and the allowance for doctors, and the army doctor salary for that rank comes to ₹89,060 a month before Dearness Allowance. This guide walks through every rank, how fast promotions come, what AFMC cadets receive while they study, and the benefits that make the uniform worth a serious look.

Key Takeaways

  • Three building blocks. An army doctor salary combines basic pay from the 7th CPC defence pay matrix, Military Service Pay of ₹15,500 a month and a Non-Practising Allowance of 20% of basic pay, plus Dearness Allowance.
  • Clear entry figures. Basic pay starts at ₹56,100 for a Lieutenant (Level 10), ₹61,300 for a Captain (Level 10B) and ₹69,400 for a Major (Level 11).
  • Fixed promotion steps. AFMS officers become Captain on completing internship, Major after 4 years as Captain and Lieutenant Colonel after 7 years as Major.
  • Benefits beyond pay. Officers get 60 days annual leave and 20 days casual leave, free medical care for the family including dependent parents and group insurance of ₹75 lakh.
  • A pay revision is coming. The 8th Central Pay Commission, set up on 3 November 2025, covers Defence Forces personnel and has 18 months to report.

What is the army doctor salary in 2026?

The army doctor salary in 2026 starts at a basic pay of ₹56,100 for a Lieutenant and ₹61,300 for a Captain, the rank a doctor reaches after internship. With ₹15,500 Military Service Pay and a 20% Non-Practising Allowance, a Captain receives ₹89,060 a month before Dearness Allowance and other allowances.

Those figures come straight from official documents. The rank and pay level of each step are listed in the AFMC Pune admission brochure, and the rupee value of each level is set by the defence pay matrix in the 7th Central Pay Commission report. A 2017 Ministry of Defence letter on pensions applies the same matrix and adds the revised Military Service Pay and Non-Practising Allowance rates notified under the 7th CPC orders, where applicable.

Here's the army doctor salary at entry level for the first three ranks:

Rank When you get it Pay level (7th CPC) Basic pay + MSP + NPA (20% of basic) Monthly total before DA
Lieutenant On commissioning 10 ₹56,100 ₹15,500 ₹11,220 ₹82,820
Captain On completing internship 10B ₹61,300 ₹15,500 ₹12,260 ₹89,060
Major After 4 years as Captain 11 ₹69,400 ₹15,500 ₹13,880 ₹98,780

The totals are our own additions of the official components. Dearness Allowance, house and transport allowances and any specialist allowances come on top and change over time, so your actual pay slip will show more than the "before DA" column.

How is an army doctor's pay built?

An army doctor's pay is built from basic pay, which depends on rank and years in that rank, plus Military Service Pay for the demands of military life, plus a Non-Practising Allowance because medical officers do not run private practice. Dearness Allowance is then paid as a percentage on top.

Let's take each part in turn.

Basic pay. This is the largest part of the army doctor salary. The 7th CPC replaced the old pay bands and grade pay with a pay matrix. Each rank has a level, and each level is a column of cells. You enter at the first cell, and every annual increment moves you one cell down the column. For a Captain at Level 10B, that means ₹61,300, then ₹63,100, then ₹65,000, and so on.

Military Service Pay (MSP). The Commission recommended MSP of ₹15,500 a month for service officers, paid to all ranks up to and including Brigadier. The report also says MSP counts as basic pay for Dearness Allowance and for pension, though not for house rent allowance or annual increments. That makes it more valuable than a flat bonus.

Non-Practising Allowance (NPA). The 7th CPC set NPA at 20% of basic pay for doctors in posts that require a medical degree, subject to a ceiling at the very top of the scale. The AFMC brochure confirms that MSP, NPA and Dearness Allowance are paid to AFMS officers "as applicable".

Dearness Allowance (DA). DA is a percentage of pay that the government revises from time to time to keep up with prices. Because it changes, we haven't put a fixed DA figure in this guide. Check the latest DA order when you compare an army doctor salary with a civilian offer.

Army doctor monthly pay before Dearness Allowance, entry level of each rank Lieutenant: basic pay 56,100 rupees, Military Service Pay 15,500, Non-Practising Allowance 11,220, total 82,820 rupees. Captain: basic pay 61,300, Military Service Pay 15,500, Non-Practising Allowance 12,260, total 89,060 rupees. Major: basic pay 69,400, Military Service Pay 15,500, Non-Practising Allowance 13,880, total 98,780 rupees. Monthly pay before DA, entry level of each rank (₹) Lieutenant 82,820 Captain 89,060 Major 98,780 Basic pay Military Service Pay NPA (20% of basic) Bar length is proportional to the amount. Dearness Allowance and other allowances are not included.

Army doctor salary by rank: the full ladder

The army doctor salary keeps rising with each promotion. The AFMC brochure lists the full rank structure for AFMS officers with the pay level of each rank:

Rank How you get there Pay level (7th CPC)
Lieutenant On commissioning 10
Captain On completion of internship 10B
Major After 4 years of service as Captain 11
Lieutenant Colonel After 7 years of service as Major 12A
Colonel By selection 13
Brigadier By selection 13A
Major General By selection 14
Lieutenant General By selection 15, 16

For Lieutenant Colonel and above we've listed only the level. The rupee value at these ranks depends on the stage each officer has reached and on later changes to the pay rules, so read it from the current pay rules rather than from a single table. What matters for planning is the shape: the first three ranks come on a fixed timetable, and every step after that is earned by selection.

How fast do army doctors get promoted?

Army doctors are promoted on a fixed timetable for the first ranks: Lieutenant on commissioning, Captain once internship is complete, Major after 4 years as Captain and Lieutenant Colonel after 7 years as Major. Colonel, Brigadier and the general ranks are reached by selection, subject to satisfactory service and fitness.

That predictability is a real plus when you plan your army doctor salary over a career. Within roughly eleven years of becoming a Captain, a doctor who keeps a good record can reach Lieutenant Colonel at Level 12A. Each promotion moves you to a higher level, and between promotions the annual increment keeps your army doctor salary moving too.

Postgraduate training also fits into this path. The AFMC brochure says AFMS officers can be selected for PG and DNB courses in service hospitals through NEET PG (or NExT once it applies), and that AFMS institutes train roughly 400 PG and 45 super-specialty students a year. Permanent commission officers can also take 2 or 3 years of study leave on full pay for advanced training in India or abroad, once they have completed the required service. If you're comparing this with the civilian route, our guide to NEET PG counselling and PG resident stipends in India shows the other side.

What do AFMC cadets get while they study?

AFMC cadets don't draw an army doctor salary while they study, but the government covers a lot. Cadets with service liability get rations and subsidised messing, hostel accommodation, a book and equipment allowance of ₹6,000 a year, uniform and other small allowances, rail warrants for vacations and insurance cover through the Army Group Insurance fund.

The AFMC Pune 2026 brochure lists 150 seats, of which 145 are for medical cadets (115 boys and 30 girls) who study MBBS with a liability to serve in the AFMS. The course is 4 and a half years plus a 12 month internship in service hospitals. After that, graduates are commissioned into the Army, Navy or Air Force, on Permanent or Short Service Commission based on merit and choice. A Short Service officer from AFMC serves at least 7 years; a Permanent Commission officer serves until superannuation. Parents sign a bond of ₹71 lakh at admission.

So the "AFMC doctor salary" people search for is simply the army doctor salary of the rank the graduate holds. Admission runs through NEET UG, and NEETsheet's guide to AFMC Pune MBBS admission after NEET 2027 covers the screening and cut-offs. Our own AFMC Pune college page has the campus details.

Is the salary the same in the Navy and Air Force?

Yes, the structure is the same. The 7th CPC report explains that officers of the Army Medical Corps are commissioned into the Army and seconded to the Navy and Air Force as each service needs them. Because they hold the same commission, they sit in the same defence pay matrix, with Military Service Pay and the doctors' allowance on the same basis.

The pay level follows the rank, not the uniform colour. That's why a search for "navy doctor salary" or "air force doctor salary" leads back to the same numbers you see in the tables above.

Benefits beyond the pay slip

A military doctor's package is much more than the monthly figure. Listed in the AFMC brochure for officers:

  • 60 days of annual leave and 20 days of casual leave each year
  • Leave Travel Concession
  • Free medical care for yourself and your family, including dependent parents
  • Group insurance of ₹75 lakh
  • Subsidised accommodation and access to the Canteen Stores Depot
  • Access to every relevant specialty and sub-specialty within the AFMS

Put together with a predictable promotion path and paid study leave for permanent officers, these benefits are the reason many families see the army doctor salary as only part of the attraction. For a broader comparison of government posts, read our guide to government jobs after MBBS, and for civilian pay across India see doctor salary in India.

Will the 8th Pay Commission raise army doctor pay?

The 8th Central Pay Commission was constituted by a Ministry of Finance resolution dated 3 November 2025. Its terms of reference include personnel of the Defence Forces, and it must make its recommendations within 18 months of its constitution. Any change to the army doctor salary will follow once the government accepts its report.

Until then, the 7th CPC matrix applies. When the new report comes out, the levels in the tables above will be mapped to new figures and the army doctor salary for every rank will move with them, so it's worth checking this page again before you make a final decision.

Planning the uniform route from MBBS abroad

Many students who don't get an AFMC seat still want a career in the forces later. MBBS graduates from other colleges can join the AFMS as medical officers through Short Service Commission entry, which our guide on how to become a military doctor explains. Read the eligibility clause of the official AFMS notification for the year you apply, because it sets the exact rules.

If you study MBBS abroad, any doctor job in India starts with the same first steps: clearing the FMGE, completing the internship in India and getting registered. Our guides to FMG internship and registration and medical registration in India after MBBS in Europe explain that sequence.

Here's a simple order of steps for either route:

  1. Qualify NEET UG. It is required for AFMC and for MBBS abroad. NEETsheet's page on NEET qualifying marks explains the cut-off you need.
  2. Choose your degree route. Pick AFMC Pune through its own screening after NEET, any recognised Indian college, or a European medical university that meets NMC rules.
  3. Finish MBBS and the internship. Complete your training and get your registration in India (for foreign graduates after the FMGE).
  4. Apply for a commission. Apply in a Short Service Commission cycle for medical officers, or take the commission that follows the AFMC course if you studied there.
  5. Plan your PG early. AFMS officers can compete for service PG seats through NEET PG.

For the wider picture, doctor salary in India on NEETsheet compares armed forces pay with central and state posts.

Your next step

A uniform career gives a doctor a clear army doctor salary, a fixed early promotion path and benefits few civilian jobs match. Whichever route you take, it starts with NEET and a recognised MBBS. If you're still preparing, NEETsheet is our app for NEET revision and mock tests.
