USMLE After MBBS From India: Steps, Fees and 2027 Dates

USMLE after MBBS from India means passing Step 1 and Step 2 CK, completing an ECFMG Pathway with an OET Medicine score, and matching into a US residency through ERAS and the NRMP. The exams, Pathway and certification alone cost about 3,700 US dollars, roughly Rs 3.5 lakh at October 2026 rates, and in the 2026 Match 56.4 per cent of non US citizen international graduates matched into a first year position.

Key takeaways on USMLE after MBBS in India

  • An Indian MBBS graduate needs ECFMG Certification before starting residency. That requires Step 1, Step 2 CK, an accepted ECFMG Pathway with OET Medicine, and a verified MBBS degree, as set out on the ECFMG certification requirements page.

  • Step 1 and Step 2 CK applications for graduates of non US schools now go through the Federation of State Medical Boards at 695 dollars each for 2026 and 2027, plus a region fee of 210 dollars for Step 1 and 235 dollars for Step 2 CK when the exam is taken outside the United States and Canada, as listed on the USMLE apply for exams page.

  • The 2027 Match calendar is fixed: ERAS opened on 2 September 2026, NRMP registration opened on 15 September, the rank order list deadline is 3 March 2027 and Match Day is 19 March 2027.

  • In the 2026 Match, 11,944 non US citizen international medical graduates took part and 56.4 per cent matched into a first year post, the NRMP results release of 20 March 2026 reports.

  • The National Medical Commission holds WFME recognition status valid to September 2033, so an NMC recognised Indian MBBS meets the ECFMG accreditation requirement that applies from 2024.

  • Each Step allows a maximum of four attempts.

What is USMLE and what does ECFMG Certification add?

The United States Medical Licensing Examination is a three step programme: Step 1 tests basic sciences, Step 2 CK tests clinical knowledge, and Step 3 tests readiness for unsupervised practice and is taken during or after the first year of residency. For a doctor trained outside the United States, the Educational Commission for Foreign Medical Graduates (ECFMG), part of Intealth, issues the ECFMG Certificate that residency programmes and state medical boards require. The certification requirements, last updated in January 2026, are four: your medical school must be listed in the World Directory of Medical Schools with an ECFMG sponsor note covering your graduation year; you must pass Step 1 and Step 2 CK; you must satisfy the clinical and communication skills requirement through an accepted, unexpired ECFMG Pathway that includes an OET Medicine score; and your final medical diploma must be verified, with at least four credit years of medical school. ECFMG Certification is also compulsory before an international graduate may sit Step 3, as the USMLE eligibility page states.

Who is eligible for USMLE after an Indian MBBS?

From 2024 ECFMG accepts only graduates of medical schools accredited by an agency recognised by the World Federation for Medical Education (WFME). India qualifies: the WFME list of accrediting agencies shows the National Medical Commission as an accrediting agency with WFME recognition status, valid to September 2033. Every NMC recognised medical college in India therefore meets the requirement, and all of them appear in WDOMS, the World Directory of Medical Schools. A student still in MBBS can take Step 1 and Step 2 CK before graduating; the certificate itself is issued only after the degree is verified. If you studied medicine in Europe rather than India, the route is the same and our separate guide on USMLE after MBBS in Europe covers the differences in document verification.

Two charges are not in the table. The OET Medicine test fee is shown only inside the OET booking portal, not on a static page, so check it when you book. The ERAS per programme application fees for the 2027 season are published by the AAMC and depend on how many programmes you apply to. Candidates testing in India also pay 18 per cent GST on the Step 1 and Step 2 CK fees, which the USMLE page says has been collected and remitted to India since 4 December 2024. Up to the Match, the fixed costs from the table come to about 3,960 dollars, around Rs 3.8 lakh, before OET, ERAS programme fees, study resources and travel.

How do you do USMLE after MBBS in India, step by step?

  1. Open a MyIntealth account on the ECFMG site and complete the online identity notarisation. Then apply for ECFMG Certification, which gives you the USMLE identification number you need to register for exams.

  2. Apply for Step 1 through the FSMB, as the USMLE apply for exams page directs non US graduates to do. Choose a three month eligibility period and book a Prometric seat in India or abroad. Step 1 has been reported as pass or fail since 2022.

  3. Apply for Step 2 CK the same way, usually after the final MBBS year or internship, because it tests clinical knowledge and its score is the number programmes compare.

  4. Book OET Medicine and score at least 350 in Listening, Reading and Speaking and 300 in Writing in one sitting, the minimums on the ECFMG OET requirements page.

  5. Apply to the ECFMG Pathway that fits you. For a graduate of an Indian college without a licence in a country that meets Pathway 1, that is usually Pathway 2 to 5, which rest on a clinical skills assessment by your medical school or on a licence. The ECFMG Pathways page sets the 2027 application deadline at 31 January 2027 for those who need it for the 2027 Match.

  6. Request your ERAS token through MyIntealth, build your MyERAS application with a personal statement, letters of recommendation and your USMLE transcript, and send it to programmes. The ECFMG ERAS page gives the 2027 season dates: applications could be started from 2 September 2026 and programmes began viewing them on 23 September 2026.

  7. Register for the NRMP Main Residency Match, attend interviews between October and January, certify your rank order list by the deadline, and wait for Match Day.

  8. After matching, apply for the J-1 visa through ECFMG sponsorship or an H-1B through the hospital, complete the ECFMG Certificate issuance, and sit Step 3 during the first year of residency.

Which documents are required for ECFMG Certification?

  • A passport, for the identity notarisation and for the name on every exam record. The name must match exactly across the passport, MBBS degree and NMC or state council registration.

  • MBBS degree certificate, or for current students a letter from the dean confirming enrolment and expected graduation.

  • Final MBBS transcript or consolidated mark sheets for all professional years, sent by the college for primary source verification; ECFMG charges 110 dollars for the diploma and 110 dollars for the transcript.

  • Internship completion certificate, because most Pathways and most programmes expect a completed internship.

  • State Medical Council or NMC registration certificate, which also serves as proof of licensure for some Pathways.

  • OET Medicine result sent directly to ECFMG by the test provider.

  • Letters of recommendation, personal statement and a medical student performance evaluation from the dean's office for ERAS.

What is the 2027 Match timeline?

The NRMP calendar for 2027 Main Residency Match applicants and the ECFMG ERAS page give these dates. All NRMP times are US Eastern Time.

The practical consequence is that a candidate who wants to match in March 2027 must have passed both Steps and completed the Pathway by 3 March 2027, and must have applied for the Pathway by 31 January. A final year student in October 2026 is therefore aiming at the 2028 Match, with Step 1 in 2027 and Step 2 CK by mid 2027, so that the ERAS application in September 2027 carries both scores.

What does the USMLE route cost a 2024 MBBS graduate aiming at the 2028 Match?

An Indian doctor needs about 5,500 dollars (around Rs 5.2 lakh) for exams, applications and visa steps, from opening her account to sitting Step 3. If she applies to 60 programmes, add roughly 1,800 dollars, which takes the total to about 7,300 dollars (around Rs 6.9 lakh). The rupee figures use Rs 95 to a dollar, as of October 2026.

When the money goes out

The heaviest stretch is April to September 2027. In those six months she pays for Step 1, Step 2 CK and the Pathway, which comes to about 2,800 dollars (around Rs 2.6 lakh). Planning for that stretch early makes the whole route easier.

What the total does not include

  • OET Medicine: She sits it in June 2027. The fee is paid separately, so add it to your own budget.

  • Interviews: She attends about ten, all online. That means no flights or hotels at this stage.

  • Study materials: Question banks and books depend on how she prepares.

  • Travel and living costs: Her trip to the exam centre in Mumbai and her move to the USA for residency come on top.

Which key terms will you meet in the USMLE process?

  • ECFMG: the Educational Commission for Foreign Medical Graduates, the body that certifies international graduates for US residency and sponsors J-1 visas.

  • ECFMG Pathways: six routes to meet the clinical skills requirement that replaced the discontinued Step 2 CS exam, each combined with OET Medicine.

  • OET Medicine: the Occupational English Test for doctors, the only English test ECFMG accepts for the Pathways.

  • ERAS: the Electronic Residency Application Service run by the AAMC, through which applications reach programmes. International graduates access it with an ECFMG token.

  • NRMP and the Match: the National Resident Matching Program pairs applicants' ranked lists with programmes' ranked lists by algorithm; results are binding.

  • SOAP: the Supplemental Offer and Acceptance Program, the four day process in Match week through which unfilled positions are offered to unmatched applicants.

  • J-1 and H-1B: the two visas under which international graduates do residency. J-1 is sponsored by ECFMG and carries a two year home residence requirement afterwards; H-1B is sponsored by the hospital and requires Step 3 before the start of residency.

What do the numbers say about your chances in the Match?

The NRMP release on the 2026 Main Residency Match reports 48,050 active applicants for 44,344 positions, of which 41,482 were filled. Non US citizen international graduates numbered 11,944 active applicants, 479 more than the year before, and 56.4 per cent of them matched into a first year position. US citizen international graduates matched at 70 per cent. Step 1 performance data on the USMLE performance data page shows that 75 per cent of first time takers from non US schools passed in 2025, against 93 per cent of first time takers from US MD schools, and 54 per cent of repeaters from non US schools passed. Read together, the figures mean that a strong first attempt on each Step and a complete application by September are what separate matched from unmatched international applicants, and that about four in ten non US citizen applicants do not match in a given year and reapply. If you are weighing the USMLE route against the United Kingdom, our guide to working in the UK after MBBS and the short web story on PLAB and GMC registration compare the entry tests, and jobs after MBBS abroad sets out the three main routes side by side.

What if you fail a Step or do not match?

If you fail a Step, the USMLE 2027 Bulletin of Information allows a maximum of four attempts per Step, with the fourth attempt at least twelve months after the first and six months after the most recent. Most programmes read a failed attempt as a weakness, so the practical rule is not to sit a Step until practice scores are comfortably above passing. If you do not match, SOAP runs from 15 to 18 March 2027 for unfilled positions, and many Indian graduates use the following year to add US clinical experience, research or observerships and reapply. If your Pathway application expires, you must reapply and pay the Pathway fee again, so time the application to the Match year you are actually targeting. If your name differs between documents, ECFMG will hold verification until the college confirms the identity, which is the single most common cause of delay for Indian applicants.

Which advanced USMLE rules do families usually miss?

  • There is no seven year rule in the USMLE programme itself. The 2027 Bulletin says only that many US state medical boards require all Steps to be completed within a set time, and that is a licensing matter decided state by state.

  • Step 1 and Step 2 CK for graduates of non US schools are applied for through the FSMB, not through ECFMG, as the USMLE page now directs. Older guides that quote an ECFMG exam fee of 1,020 dollars describe the 2025 arrangement.

  • The ECFMG Pathway must be unexpired at the time of the Match. Apply for the Pathway in the season you will actually match, not a year early.

  • ECFMG Certification is not required to submit an ERAS application, but the NRMP requires international applicants to have met the ECFMG examination requirements by the rank order list deadline of 3 March 2027.

  • The NRMP charges 30 dollars for each programme ranked beyond 20, with a maximum of 300 ranks, and a length of list fee that rises from 50 dollars for 100 to 150 ranks to 200 dollars for 251 to 300 ranks, as listed on the NRMP fees page.

  • Step 3 must be passed before an H-1B visa can be used for residency, which is why most Indian graduates on an H-1B take Step 3 before the Match.

What are the alternatives if USMLE is not the right fit?

Postgraduate training in India through NEET PG, the United Kingdom through PLAB and GMC registration, Australia through the AMC exams, the Gulf through DHA, MOHAP, DOH and SCFHS licensing, and specialist training in Germany after the Fachsprachprüfung are all live routes for an Indian MBBS graduate. Our overview of which exam is required after MBBS abroad compares them in one table, and NEETsheet's guides to medical specialisations and doctor salaries in India help compare the Indian PG route against three to seven years in the United States.
