The University of Cape Town Faculty of Health Sciences, Africa's leading medical school and the site of the world's first heart transplant in 1967, offers an MBChB registrable with the Health Professions Council of South Africa (HPCSA) and listed in the World Directory of Medical Schools. It does not meet the National Medical Commission's rules for practising in India (see FMGE).

How to Apply
Admission to the UCT MBChB is on academic merit plus the National Benchmark Tests (NBT), with limited international places. UCT publishes the current process and international quota on uct.ac.za.
International school-leavers must have a strong school-leaving qualification (equivalent to the South African NSC) with the required subjects and a high aggregate; UCT requires a high APS (a sub-minimum of 450 was cited) for the MBChB.
All Health Sciences applicants, including internationals, must write the National Benchmark Tests (NBT); the MBChB requires NBT scores in the 'Proficient' range.
Apply through UCT's admissions process, within the (limited) international MBChB quota, meeting Department of Home Affairs requirements for foreign students.
Selection is on academic merit, NBT results and any additional requirements; meet English-proficiency requirements (IELTS/equivalent) as needed.
Successful applicants receive an offer and apply for a South African study visa, showing proof of financial means.

The UCT MBChB is a six-year programme taught in English, from pre-clinical foundations to full-time clinical training at Groote Schuur and other Cape Town teaching hospitals. The year-by-year outline below is representative; UCT publishes the current structure on health.uct.ac.za.

Clinical training takes place at Groote Schuur Hospital, the site of the first heart transplant, and other Cape Town teaching hospitals. In South Africa, graduates complete a two-year internship and a year of community service to register with the HPCSA. This is not the 12-month internship at the same university that the NMC's FMGL Regulations 2021 ask for, so the degree does not lead to practice in India. An indicative pattern is:

Legacy
The University of Cape Town has one of the most remarkable medical legacies in the world, from the first heart transplant to the theory behind the CT scan and a yellow-fever vaccine. Where someone is famous as a UCT professor rather than only a graduate, that is stated plainly.
Studied medicine at UCT and became a cardiac surgeon at Groote Schuur Hospital, where on 3 December 1967 he led the team that performed the world's first human-to-human heart transplant, opening the era of organ transplantation.
Christiaan's younger brother, a UCT-trained cardiac surgeon and a key member of the first heart-transplant team in 1967; he later helped create the world's first dread-disease (critical illness) insurance.
Earned his physics degrees at UCT and lectured there; his work at UCT and Groote Schuur in the 1950s produced the mathematical theory behind X-ray computed tomography, winning the 1979 Nobel Prize in Physiology or Medicine.
Began his medical studies at UCT before continuing in London and the United States, where he developed the yellow-fever vaccine. The work that earned him the 1951 Nobel Prize in Physiology or Medicine.
Studied science at UCT before his career in molecular biology in the UK; awarded the 1982 Nobel Prize in Chemistry for crystallographic electron microscopy and the structures of viruses and nucleic-acid–protein complexes.
A distinguished UCT cardiologist and Dean of the Faculty of Health Sciences from 2016 until his death in 2018; a world leader in rheumatic heart disease and the genetics of cardiomyopathies.

Cape Town has a strong Indian and Cape Malay food heritage, with many Indian restaurants and curry houses, so Indian food, including familiar regional dishes, is easy to find.
Cape Town has a large Muslim community and widely available halal food, Cape Malay cuisine especially, so Muslim students are very well catered for.
Cape Town has excellent vegetarian and vegan options, with dedicated restaurants and clearly labelled menus, so vegetarian students eat very well.
Indian and Asian grocery stores in Cape Town stock spices, dal, rice, atta and ready-mixes, and many deliver, so students can cook familiar food.

Practising in India
A South African MBChB does not meet the NMC's rules for practising in India. The six-year course is long enough, but graduates then do a two-year internship at accredited hospitals and a year of community service (South African Government), not the 12-month internship at the same university that the NMC's FMGL Regulations 2021 ask for. Foreign nationals are placed in internship posts only after all South African citizens and permanent residents, and only 2 foreign nationals were placed between 2019 and 2024 (parliamentary reply, 7 March 2024). Without that internship there is no full HPCSA registration. South Africa is also not covered by the FMGE exemption for postgraduates from Australia, Canada, New Zealand, the UK and the USA.
Step 1
NEET UG is not part of admission here, and because this degree does not meet the NMC's rules, NEET UG does not open a route to practise in India from it.
Step 2
The NMC's Foreign Medical Graduate Licentiate Regulations 2021 need at least 54 months of study plus a 12-month internship at the same university, taught in English, and a licence to practise in the country of study on the same terms as its citizens. In South Africa the internship is a two-year hospital post, and foreign nationals are placed only after citizens and permanent residents, with just 2 placed from 2019 to 2024, so there is no full HPCSA registration either.
Step 3
The NBEMS FMGE bulletin says anyone admitted abroad on or after 18 November 2021 whose degree does not meet the FMGL rules cannot sit the FMGE, so this degree does not lead to NMC or State Medical Council registration.

Fees & Financial
A genuine advantage of UCT is affordability: South African tuition and living costs are far below Australia, New Zealand or the UK. Honest financial planning still matters, as the international fee is higher than the domestic one.
In 2026 the MBChB costs students from outside Africa about R199,500 a year (about ₹11.5 lakh), but South African tuition and living costs remain a fraction of the Australia/NZ/UK level, so UCT can be a genuinely affordable route to a world-class, English-taught degree. UCT publishes the international MBChB fee on uct.ac.za.
UCT and South African bodies offer some scholarships and financial aid, but most are aimed at South African and African students, and medicine funding for internationals is limited. Eligibility for international MBChB students varies by scholarship.
Because South Africa is comparatively affordable, the funding requirement is lower than for Australia or New Zealand, but families should still plan carefully. We guide families realistically on costs, loans and whether the numbers make sense, without pushing anyone toward a decision they cannot afford.
